REGISTRATION

Admission Process

Mt Carmel Preschool & Daycare serves children from eighteen months to five years of age. No child will be denied admission on the basis of race, religion, or handicap. We will do our best to accommodate children with special needs by taking necessary measures needed to meet the needs of the child.  We do retain the right to deny admission if all slots for a particular classroom are filled. Your child can be placed on a waiting until a spot is available.  To begin the enrollment process, you must pay a one-time registration fee of $70.00 (non-refundable) when the application is submitted. If your child enrolls into one of MCBC Daycare’s classroom, the $70 fee will be credited towards your child’s account. Once a slot opens up, the next on the waiting list will have the first opportunity to fill the slot or it will then go on the next child on the waiting list.  Should your child be accepted and enrolled into one of our classrooms, first payment is due prior to the agreed upon start date.

Each of the following forms must be completed in order for your child to begin school.

Please complete one set of forms per child enrolled.

Please download, complete, and print these Mt Carmel Preschool & Daycare forms and return them to the Director by the first day of school

  1. Registration Form
    Fill out this form first prior to acceptance
     
  2. Child/Family Information Form
     
  3. Transportation Authorization Form
     
  4. Photo Release Form

Please download, complete, and print these Ohio Dept. of Job and Family Services (ODJFS) required forms and return them to the Director by the first day of school
(Forms may be filled out electronically or manually)


  1. Child Enrollment and Health Information Form
    This form is different from the registration form
     
  2. Child Medical Statement Form
    Must be updated every 12 months, signed by a doctor, and accompanied by current immunization records.
